Possibly this problem is caused by another driver on your system which cannot be identified at this time. Until now! I dont want to go any further just incase i cause more damage. This usually indicates problem with the device itself or with the device driver programming the hardware incorrectly.
But it is always unpredictable as to when it happens and what event caused it.
